Effective July 1, 2021, Key Risk is the Worker's Compensation provider for the District (insured by StarNet). Located below are the forms you will need to report an accident (Incident Report Form) and the packet you will need after sustaining an injury. The incident report form should be completed at the time of any incident and submitted to your direct supervisor, the school nurse in your building, or the Superintendent's Office.
The post injury packet includes the following documents:
- Authorization Form (you should complete this form and send it to Key Risk - signing this forms gives your medical provider authorization to release your medical information to our Worker's Comp provider)
- Letter of Introduction to your medical provider
- Physician Report (after you complete the first part of the form - give this to your medical provider to provide information on your injury and if there will be any restrictions on your return to work)
- Temporary Pharmacy Card (SmithRx). If your medical provider prescribes medication, you can use this temporary pharmacy card to fill your prescription. This pharmacy card is accepted at most of the big pharmacies (Rite Aid, Walgreens, Walmart, Giant Eagle . . .) as well as at Spartan Pharmacy. This card is good for one time use only.
Another new feature with Key Risk will be your ability to speak with a health care provider virtually. More information on this feature will be coming soon.
