Effective July 1, 2025, Workpartners is the Worker's Compensation provider for the District.  The Claim Kit has all of the forms you will need to report an accident and the packet you will need after sustaining an injury.

The Claim Kit has the Physician Panel, First Incident Report Form (Report of Injury), Claimant Questionnaire, Authorization for Release of Protected Health Information, and a temporary prescription card.  

The incident report form should be completed at the time of any incident and submitted to your direct supervisor, the school nurse in your building, or the Superintendent's Office.
